Sports Physicals

Middle school and high school athletic programs, and some private sports clubs, require a sports physical exam and form completion in advance of participating in an athletic program; the purpose of these exams is to provide your child or adolescent a comprehensive evaluation to identify any at-risk concerns for possible heart, lung, orthopedic, or other medical conditions that may require further attention.

Please print the Preparticipation Sports Physical Evaluation Form (child and/or parent should complete the history section on page 1) and also print the Pediatric Sudden Cardiac Death Risk Assessment Form and bring these with you to your office exam.
