How to transfer medical records to the Pediatric Place
There are two options for you to choose:
(1) Go to Forms and select the form Request for Medical Records; bring or fax this document to your child’s previous medical practice; it may take up to 30 days until we receive medical records; please call to confirm we have received your child’s records
(2) Call your previous medical practice and request a copy of their own medical release form; provide the practice with my office info – 9570 Nesbit Ferry Rd. Suite 201

How to obtain a copy of my child’s medical records from The Pediatric Place
Go to Forms and select the form Release of Medical Records. Mail, fax, or bring this form to my office and we will be able to provide you with a copy of your child’s medical records within 7-10 days or we can mail these records directly to another physician’s office. We charge a printing fee for a copy of your child’s medical records.
