School Vaccine Requirements

Vaccine Schedule-

My practice adheres to the Center for Disease Control and Prevention (CDC) standard childhood recommended vaccine schedule. School Requirements (Georgia)–

Children entering Kindergarten must have completed all CDC-recommended early childhood vaccines including 2-doses of MMR (measles-mumps-rubella), 2-doses of Varivax (chicken pox), and 2-doses of Hepatitis A vaccines

Children entering Middle School (6th grade) must have completed the vaccines listed above as well as a booster of Tetanus (usually provided as Tdap vaccine); in addition, while not required in Georgia, it is highly recommended that your child receive the Meningococcal vaccine (single-dose only)

Form 3231 (printed by the practice from data entered into the GRITS-Georgia Immunization Registry) is required for entrance into day care, preschools, kindergarten and middle school as well as for any child who is newly enrolled in school in the State of Georgia
